TCM supports your independence, your goals, and your life
Navigating service systems can feel overwhelming. Waitlists, paperwork, appointments, eligibility rules…it’s a lot to manage on your own. Targeted Case Management (TCM) is here to make that journey easier, clearer, and more connected.
Omni Advocacy Services is an approved TCM provider through local Kansas CDDOs, serving Kansas City, Johnson County, and Topeka. If you or your family member receives IDD services, and are on your counties' CDDO waitlist, you can choose Omni as your Targeted Case Management provider.
Targeted Case Management (TCM) is a Medicaid‑funded service that helps people access the supports they need to live, work, and participate fully in their communities. Your TCM works alongside you to understand your goals, identify resources, coordinate services, and advocate for your needs across systems.
Targeted Case management assists individuals eligible for services in accessing comprehensive medical, social, educational, and other specialized services. Case managers are trained in mental health, social work or closely related fields and work with the individual to identify, locate, coordinate and monitor services that meet the individual's distinct need.
Case managers work in many aspects of people's lives, and may work with children and families, senior citizens, those with disabilities, recovering addicts, the mentally ill, immigrants, ex-offenders or the homeless. Each of these jobs have their own specific needs and requirements.
A Targeted Case Manager will assist you by providing support, guidance, and encouragement along the way. Service systems can be confusing and complicated at times; it can help to have a TCM who can act as an advocate, understands available resources, and can help guide your path forward.

Apply for services or benefits
Understand your options through the CDDO
Coordinate medical, behavioral health, or community supports
Problem‑solve barriers to independence
Prepare for life transitions (new housing, job changes)
Communicate with providers or systems on your behalf
Every person’s needs are different, and your TCM’s role adapts to fit your life. You aren't given a generic checklist. You and your manager collaborate on a care plan that matches your specific goals, such as finding a stable home or finishing a degree.
Beyond just finding resources, TCM helps you learn essential life skills, like budgeting or managing medications, so you eventually feel more in control of your own life.
To receive TCM services, you must be:

On the CDDO waitlist
If you are not yet on the waitlist, we can help you get started.
Once you are on the waitlist or approved for services, you can select Omni Advocacy Services as your TCM provider.
You can receive both Supported Employment (SE) and Targeted Case Management (TCM). They serve different purposes and complement each other.
